Role Description
This is a full-time on-site role for a Care Manager, as a Care Manager, you will be responsible for overseeing and coordinating the care services provided to their clients. Your day-to-day tasks will include conducting assessments, developing care plans, coordinating with healthcare professionals, managing a team of carers, and ensuring the highest quality of care for their clients. This role requires strong organizational and leadership skills.
The Person
- Case Management and Elder Care experience
- Training skills in the healthcare field
- Knowledge of Social Services and support systems
- Background in Medicine or healthcare-related fields
- Excellent communication and interpersonal skills
- Ability to work well under pressure and problem-solve
- Experience in care coordination and scheduling
- Familiarity with regulatory requirements and compliance
